Terms and conditions for Couple Paid in full 6 month membership

On month-to-month memberships your debit will continue until we receive a 5 DAY written notice from you to either freeze or cancel your membership.
MEMBERSHIP DOES NOT AUTOMATICALLY CANCEL.
Terms: By my signature below, this authority is to remain in effect until a 5 day advance written notice is given to Patriot Sports Performance to either freeze or cancel the membership. I understand that I will incur charges to my account should prior notice not be given to Patriot Sports Performance and that I may also risk additional late fees should my account become delinquent. For the written notice to become effective, the member must complete and sign in the presence of a Patriot employee, a Membership change form. When a membership is FROZEN, a $15 per month freeze fee will be debited from my account on file until further notice is given and my activity is suspended until so. All monies owed are still the responsibility of the member. If you are PAST DUE on your membership dues a $15 late charge will be assessed. If Patriot Sports Performance is unable to collect monies owed on the account after 3 attempts, the membership will be terminated.
Waiver and Release of Liability: Patriot Sports Performance urges you and all members to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise class or sports program. All classes, games and exercises, including weights and use of any and all machinery, equipment, and apparatus designed for exercising shall be at the member’s sole risk. Member understands that the agreement to use, or selection of exercise programs, methods and types of equipment shall be member’s entire responsibility, and Patriot Sports Performance shall not be liable to member for any claims, demands, injuries, damages, or actions, or actions arising due to injury to members person or property arising out of or in connection with the use by member of the services, facilities, and premises of Patriot Sports Performance. Member also acknowledges that they can come in contact with all sorts of bacteria, viruses including Covid-19 and that they do not hold Patriot Sports Performance liable for any contact, sickness or death resulting from such viruses, bacteria and diseases. Member hereby holds Patriot Sports Performance and it’s officers, owners, agents, contractors and employees harmless from all claims which may be brought against them by member or on member’s behalf for any such injuries or claims.
